What is Duavee and how does it work?
Duavee is a tissue-selective estrogen complex (TSEC) that pairs two ingredients: conjugated estrogens (an estrogen) and bazedoxifene (a selective estrogen receptor modulator, or SERM). The FDA has approved it for postmenopausal women who still have a uterus to treat moderate-to-severe vasomotor symptoms (hot flashes) associated with menopause and to help prevent postmenopausal osteoporosis.
The bazedoxifene component is what makes Duavee different from plain estrogen therapy. It protects the lining of the uterus (the endometrium) from estrogen-driven overgrowth, which is why a separate progestin is not needed. Duavee is meant only for women who still have a uterus. It should be used at the lowest effective dose and for the shortest time that fits your treatment goals. The Women's Health Initiative (WHI) studies of estrogen alone and estrogen plus progestin reported higher risks of stroke, DVT, and probable dementia in women 65 and older; bazedoxifene may also add to the risk of venous blood clots.
Duavee should not be used if you have undiagnosed abnormal vaginal bleeding; a known, suspected, or past history of breast cancer or other estrogen-dependent tumors; a current or past history of blood clots (DVT/PE) or arterial clot-related events such as stroke or heart attack; liver disease or impairment; a known clotting (thrombophilic) disorder; or known or suspected pregnancy. Do not combine it with additional estrogens or progestins. Stop the medicine and call your provider right away if you have a blood clot, stroke, heart attack, or vision problems.
