What is econazole nitrate and how does it work?
Econazole nitrate is a prescription topical azole (imidazole) antifungal. As a 1% cream, it is FDA-approved to treat several common fungal skin infections caused by dermatophytes (Trichophyton, Microsporum, and Epidermophyton species), including tinea pedis (athlete's foot), tinea cruris (jock itch), and tinea corporis (ringworm). It is also approved for cutaneous candidiasis (a yeast skin infection) and for tinea versicolor. A separate 1% topical foam product, Ecoza, is approved specifically for interdigital (between-the-toes) athlete's foot in patients 12 years and older.
Like other azole antifungals, econazole works by interfering with the fungal cell membrane, which stops the fungus from growing. It is for external skin use only. The original brand cream, Spectazole, has been discontinued in the US, but the generic remains widely available.

Safety and side effects
Econazole nitrate has no boxed warning. It is for topical, external use only — not for use in the eyes, mouth, or vagina. The most common side effects are local reactions at the application site, such as burning, stinging, itching, redness, or irritation. Stop using it and contact your provider if you develop signs of sensitization or worsening irritation. If you are using the Ecoza foam, note that it is flammable — avoid fire, flame, and smoking during and right after you apply it.
Give the medicine time to work: tinea versicolor and candidal (yeast) infections may improve within about 2 weeks, while dermatophyte infections such as athlete's foot may need up to a month of treatment to reduce the chance of recurrence. Systemic absorption is low with topical use, but use during pregnancy only if clearly needed.
