What is hydroxyurea and how does it work?
Hydroxyurea is an antineoplastic (anti-cancer) medicine in a class called antimetabolites. It works as a ribonucleotide reductase inhibitor, which blocks an enzyme cells need to make DNA and slows the production of certain blood cells. It is FDA-approved to reduce the frequency of painful crises and the need for blood transfusions in adults (sold as Droxia) and in children (sold as Siklos) with sickle cell anemia, to treat resistant chronic myelogenous leukemia (CML), and, given together with radiation, to treat certain head and neck cancers (sold as Hydrea).
Hydroxyurea is taken by mouth, usually once a day, and your dose is adjusted based on how you respond and on your blood test results. This page is educational only and is not medical advice; use hydroxyurea exactly as your prescriber directs.
How much does hydroxyurea cost without insurance?
Hydroxyurea is available as an FDA-approved generic, so it is generally one of the more affordable medications in its class. Even so, the cash price is not the same everywhere. Because pharmacies set their own prices, the amount you would pay without insurance can differ significantly between two stores in the same town, which is exactly why comparing is worth the few seconds it takes.
Rx.com compares hydroxyurea prices across more than 60,000 U.S. pharmacies and shows you a free discount coupon you can use whether or not you have insurance. Enter your ZIP above to see today's price near you. Hydroxyurea does not have a manufacturer copay savings card, so a pharmacy discount coupon is often the simplest way to lower the cash price on the generic.
Brand versions and related medicines
Hydroxyurea is the active ingredient in several brand-name products, and the generic is substitutable for them at the pharmacy. Related pages you may find helpful include Hydrea (used with radiation for certain head and neck cancers and for resistant CML) and Siklos (the tablet approved for sickle cell disease in children). Because these are all forms of the same drug, the generic is usually the lowest-cost way to fill the prescription.
For sickle cell disease, hydroxyurea is often part of a broader treatment plan. Another oral option your care team may discuss is Endari (L-glutamine). These medicines work differently and are not interchangeable, so which one is right for you is a decision for your hematologist.
Hydroxyurea safety and monitoring
Hydroxyurea carries FDA boxed warnings. It can cause severe myelosuppression, meaning dangerously low white cell, red cell, and platelet counts, so your provider will check your blood counts before you start and monitor them throughout treatment; it should not be started if your bone marrow function is already markedly reduced. The boxed warning also notes that hydroxyurea is carcinogenic with long-term use, raising the risk of secondary leukemias and skin cancer, so sun protection and regular skin checks are advised.
Hydroxyurea can also cause serious harm to an unborn baby, so effective contraception is advised for both women and men of reproductive potential. Other possible effects include skin (vasculitic) toxicities and leg ulcers, enlarged red blood cells (macrocytosis), and a higher risk of infection. This is not a complete list; talk with your provider or pharmacist about your full health history and any other medicines you take.
