How Much Is Zepbound Without Insurance? Cost by Dose in 2026

Zepbound has a list price of up to $1,086.37 a month, but almost nobody without insurance pays that. Here is what each dose really costs through Lilly's self-pay program, how the on-time refill rule works, and the cheaper paths you may qualify for.

Medically informed guide · Updated October 2026 · 9 min read

Without insurance, Zepbound costs $299 to $699 a month through LillyDirect, Lilly's own pharmacy, depending on your dose. As of October 2026, the self-pay vial and KwikPen cost $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg through 15 mg if you refill within 45 days (otherwise $499 for 7.5 mg and $699 for 10 to 15 mg). Lilly's published list price is $499 to $1,086.37 per 28-day fill, which is what the drug costs before discounts, not what most people pay.

  • List price: $499 to $1,086.37 per 28-day fill (Lilly, list price as of January 1, 2026).
  • LillyDirect self-pay (vial or KwikPen): $299 to $449 a month with on-time refills.
  • The $449 price for 7.5 mg to 15 mg needs each refill within 45 days of your last delivery.
  • Commercial insurance that covers the pen plus the savings card: as low as $25 a month.
  • Eligible Medicare Part D patients: $50 a month through the Medicare GLP-1 Bridge (KwikPen only).
  • Zepbound is not on the Lilly Cares patient assistance list as of October 2026.
Bottom line: If your insurance does not cover Zepbound, the LillyDirect self-pay price ($299 to $449 a month with on-time refills) is the number to beat. Check insurance and Medicare options first, because they can be far cheaper.

Zepbound cost by dose (October 2026)

Zepbound comes in three forms: a single-dose pen, a single-dose vial, and a KwikPen that holds four weekly doses. Lilly sells the vial and the KwikPen to cash-paying patients at a set price for each dose. A "month" in Lilly's terms is 28 days: four vials or one KwikPen (Lilly, Zepbound coverage and savings).

*List price (wholesale acquisition cost) is the range Lilly publishes for Zepbound per fill, as of January 1, 2026. Self-pay prices are as of October 2026. "On time" means you complete each refill within 45 days of the delivery or receipt date of your last vial or KwikPen fill. Taxes and fees may apply. Sources: Lilly pricing information, Lilly savings terms.

A few details matter here. The vial is sold only through LillyDirect Pharmacy, with home delivery or pharmacy pick-up. The KwikPen can be filled through LillyDirect or another pharmacy with Lilly's KwikPen self-pay savings card. Pen needles for the KwikPen are sold separately, so add those to your budget.

The FDA label starts everyone at 2.5 mg once a week for 4 weeks, then raises the dose in 2.5 mg steps at least 4 weeks apart. For weight loss, the maintenance doses are 5 mg, 10 mg, or 15 mg. For sleep apnea, they are 10 mg or 15 mg (DailyMed, Zepbound label). The 2.5 mg dose is only for getting started. Our tirzepatide dosage guide walks through each step.

The 45-day on-time refill rule

The $449 price for 7.5 mg, 10 mg, 12.5 mg and 15 mg comes from Lilly's Zepbound Self Pay Journey Program. You get it automatically on your first purchase at one of those doses. To keep it, you must complete each refill within 45 days of the date your last vial or KwikPen fill was delivered or received (Lilly purchase offer terms).

If you go past 45 days, the regular price applies: $499 for 7.5 mg and $699 for 10 mg, 12.5 mg or 15 mg. At the higher doses, one late refill costs you $250 more for that month.

💡 How to stay inside the window

Each fill is a 28-day supply, so you have about two weeks of slack. Set a reminder to reorder around day 21. If you are traveling or pausing treatment, talk to your prescriber before the 45 days run out.

Why the list price and what you pay differ

The list price, also called wholesale acquisition cost, is the price Lilly charges wholesalers. Lilly says what patients pay "may be different" and depends mostly on insurance, pharmacy charges, and discounts (Lilly pricing information).

The LillyDirect self-pay price is a separate price that Lilly sets for people paying cash. It is not the list price, and you cannot run it through insurance or count it toward a deductible. That is why the same box can cost $1,086.37 on paper and $449 at LillyDirect.

1. Insurance coverage plus the savings card

If your commercial plan covers the Zepbound single-dose pen, the savings card can bring your cost to as little as $25 for a 1-, 2- or 3-month fill. Savings are capped at $100 per 1-month fill and $1,300 per calendar year, for up to 13 fills. The current card expires December 31, 2026 (Lilly savings terms). Ask your plan if it needs a prior authorization first.

2. Medicare GLP-1 Bridge (KwikPen only)

From July 1, 2026 through December 31, 2027, eligible Medicare Part D members pay a $50 copay for certain GLP-1 drugs used for weight management (CMS, Medicare GLP-1 Bridge). For Zepbound, only the KwikPen is included. CMS says the single-dose vial and single-dose pen "will not be available" through the Bridge (CMS, information for providers).

To qualify, you must be 18 or older, be prescribed it for weight loss along with lifestyle changes, and meet one of these at the start of treatment: a BMI of 35 or higher; a BMI of 30 or higher with heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease stage 3a or higher; or a BMI of 27 or higher with prediabetes, a past heart attack, a past stroke, or symptomatic peripheral artery disease. Prescriptions for sleep apnea go through your regular Part D plan instead. Your prescriber submits a prior authorization, and pen needles are not covered. You can check eligibility at Medicare.gov/glp1bridge or by calling 1-800-MEDICARE.

3. LillyDirect self-pay

If you have no coverage, the vial or KwikPen at $299 to $449 a month is usually the lowest cash price for the brand. No prior authorization is needed, but you must certify that the prescription is for an FDA-approved use. Savings cannot be billed to insurance or applied to a deductible.

4. Employer plans, HSA and FSA

Some employer plans cover Zepbound for weight loss or sleep apnea, and some exclude it. Ask HR or your plan for the formulary and prior authorization rules. If you pay cash, the IRS treats prescribed drugs as qualified medical expenses, and weight-loss treatment counts when a doctor has diagnosed a disease such as obesity (IRS Publication 502). That can make HSA or FSA money usable for what you pay out of pocket. One limit: you cannot claim the amount the savings card covered. Confirm with your account administrator.

5. Patient assistance (Lilly Cares)

The Lilly Cares Foundation gives free Lilly medicines to people who qualify. As of October 2026, Zepbound is not on its list of available medications (Lilly Cares), so it is not an option for this drug right now.

What the savings card does and does not do

The Zepbound savings card is easy to misread. Here is what Lilly's terms say (Lilly savings terms):

  • With pen coverage: as low as $25 per fill, with a $100 monthly cap. If your copay is $300, the card takes off $100 and you pay $200.
  • Without pen coverage: as low as $499 for a 1-month fill of the single-dose pen. That is more than the vial or KwikPen self-pay price at every dose.
  • KwikPen without coverage: as low as $299 (2.5 mg), $399 (5 mg) or $449 (7.5 mg to 15 mg, with on-time refills), for up to 11 fills a year.
  • Vials: the card does not apply. Vial savings are applied automatically at LillyDirect.
  • Government insurance: anyone on Medicare, Medicaid, TRICARE, VA or another government plan cannot use the card.
  • No stacking: the card cannot be combined with any other discount card, coupon or offer.
  • Expiry: the current card ends December 31, 2026.

Do discount cards beat LillyDirect?

Pharmacy discount cards and coupons are worth checking for many drugs, but for Zepbound you should compare any coupon price against the LillyDirect price for your dose: $299, $399 or $449 a month with on-time refills. Lilly's self-pay prices are already far below the list price, and the savings card cannot be combined with a discount card. If a coupon price is higher than the LillyDirect number, LillyDirect is the cheaper choice.

What Zepbound costs over a year

Lilly counts a month as 28 days, so a year is 13 fills. Here is the math for common situations as of October 2026. It does not include pen needles, taxes or the cost of your provider visits.

SituationMathYearly drug cost
Covered pen + savings card, copay within the cap13 x $25$325
Medicare GLP-1 Bridge (KwikPen)13 x $50$650
Self-pay, 5 mg maintenance all year13 x $399$5,187
Self-pay, first year: 2.5 mg, 5 mg, 7.5 mg, then 10 mg on time$299 + $399 + 11 x $449$5,637
Self-pay, 10 or 15 mg maintenance, on time13 x $449$5,837
Self-pay, 10 or 15 mg, refills always late13 x $699$9,087
Top of the list-price range13 x $1,086.37$14,122.81

Two things stand out. On-time refills save $3,250 a year at 10 or 15 mg. And 5 mg is a full maintenance dose for weight loss, so if it works for you, it is the lowest self-pay cost. Your prescriber decides your dose based on your results and side effects, not on price alone. 🚨 When to Contact Your Healthcare Provider

Whatever you pay, do not stretch doses or skip weeks to save money without talking to your prescriber. Get medical help right away for any of these warning signs listed on the label. Call 911 for trouble breathing or fainting.

  • Severe stomach pain that will not go away, with or without vomiting, which can spread to your back: possible pancreatitis.
  • Pain in your upper stomach, fever, yellow skin or eyes, or clay-colored stools: possible gallbladder problems.
  • Nausea, vomiting or diarrhea that does not stop: risk of dehydration and kidney problems.
  • Swelling of the face, lips, tongue or throat, or a severe rash: possible allergic reaction.
  • A lump or swelling in the neck, hoarseness or trouble swallowing: tell your provider.

Frequently Asked Questions

How much is Zepbound without insurance?

Through LillyDirect, the vial or KwikPen costs $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg to 15 mg with refills within 45 days, as of October 2026. Late refills cost $499 (7.5 mg) or $699 (10 to 15 mg).

What is the list price of Zepbound?

Lilly publishes a list price of $499 to $1,086.37 per 28-day fill, as of January 1, 2026. Few people pay that, because self-pay, insurance and savings card prices are lower.

Is the Zepbound vial cheaper than the KwikPen?

No. Self-pay prices are the same for the vial and the KwikPen at every dose. The vial is sold only through LillyDirect, and the KwikPen needs pen needles, which are sold separately.

Does Medicare cover Zepbound?

Eligible Part D members can get the Zepbound KwikPen for a $50 copay through the Medicare GLP-1 Bridge, July 1, 2026 to December 31, 2027, if prescribed for weight management and BMI rules are met. The vial and single-dose pen are not included.

Can I use the Zepbound savings card without insurance?

Not for the single-dose pen at the $25 price, which needs coverage. Cash-paying adults can use the KwikPen self-pay savings card, and vial savings apply automatically at LillyDirect. The card cannot be used with government insurance.

What happens if I refill Zepbound late?

If you refill more than 45 days after your last vial or KwikPen delivery, the $449 offer for 7.5 mg to 15 mg does not apply, and you pay the regular $499 or $699 price.

Can I use my HSA or FSA for Zepbound?

Usually, yes, for the amount you pay. The IRS counts prescribed drugs as medical expenses. You cannot claim the part the savings card paid. Check with your account administrator.

Does Lilly Cares cover Zepbound?

Not as of October 2026. Zepbound is not on the Lilly Cares list of available medications.
