This is a compounded medication and is not FDA-approved. Its safety and effectiveness have not been verified by the FDA. Compounded medications are prepared by licensed compounding pharmacies under Section 503A of the Federal Food, Drug, and Cosmetic Act, based on a valid patient-specific prescription from a licensed healthcare provider. Rx.com does not manufacture compounded medications.

Cost

How much does compounded tirzepatide cost?

Compounded tirzepatide through Rx.com is $149 a month on the 12-month plan, or $285 month to month. That price covers your online provider visit, the medication, supplies and cold-packed shipping, and it stays the same when your dose goes up.

Price by plan

PlanTotalPer month

Per-month amounts are the weekly dose × 52 weeks ÷ 12 months. Price shown is the 12-month plan. Your provider decides your starting dose and when to step up.

Tirzepatide is started at 2.5 mg once a week and usually stepped up about every 4 weeks. At 15 mg you use roughly 6 times as much medication each month as at the starting dose, so a price quoted per milligram or per vial keeps rising as you titrate. With one flat price, stepping up never changes your bill.

Before you prepay for any tirzepatide plan, ask

  • Does the price change when my dose goes up? Per-milligram pricing gets more expensive every few weeks.
  • Is the provider visit included? Some programs add an intake fee or a monthly membership.
  • Are supplies and cold shipping included? Syringes, alcohol pads and cold-packed delivery can be billed separately.
  • How much ships at once? An opened multi-dose vial has a beyond-use date, so a large shipment only saves money if you can use it in time.

How it works in the body

How tirzepatide works

Tirzepatide is a GLP-1 / GIP dual receptor agonist. Taken once a week, it works with your body's own appetite and blood-sugar signals.

  • Feel full soonerIt slows how quickly food leaves your stomach, so meals keep you satisfied longer and portions naturally shrink.
  • Quiet the cravingsIt acts on appetite centers in the brain, turning down hunger and the constant pull toward snacking.
  • Steadier blood sugarIt helps your body release insulin when blood sugar rises, which supports more stable energy through the day.

In the 72-week SURMOUNT-1 trial of FDA-approved tirzepatide (Zepbound), adults on the 15 mg dose lost an average of about 21% of their body weight alongside diet and exercise. Individual results vary, and compounded tirzepatide has not been studied in those trials. Tirzepatide works best alongside a reduced-calorie diet and regular activity.

The basics

What is compounded tirzepatide?

What compounded tirzepatide is

Compounded tirzepatide is tirzepatide prepared by a licensed compounding pharmacy for one patient, from a prescription written by that patient's provider. It contains the same active ingredient studied in the clinical trials, but the finished product is made by the pharmacy rather than a drug manufacturer, so it has not been reviewed by the FDA for safety, effectiveness or quality. Rx.com prescribes it as a weekly injection, not a pill or drops.

Is compounded tirzepatide legal?

Yes, when it is prescribed for you. The FDA declared the tirzepatide shortage over in December 2024. State-licensed 503A pharmacies may still compound it for an individual patient with a valid prescription when the prescriber decides a compounded version meets that patient's medical need. The FDA has warned about products sold without a prescription, products labeled “for research use only” and products that are not made by a licensed pharmacy.

Is compounded tirzepatide safe?

The active ingredient has a known safety profile, with the same boxed warning and side effects described below. The added risks with any compounded medication come from how it is made and used: the FDA has received reports of dosing mistakes when people confused milligrams, milliliters and syringe markings, and it has warned about versions made with salt forms of the drug, which are different active ingredients. That is why your prescription, your pharmacy label and the syringe your pharmacy sends matter.

How to tell a legitimate source

A prescription from a licensed provider after a medical review. A pharmacy licensed in your state that you can look up on your state board of pharmacy website. A label with the drug, strength and pharmacy name. Cold-packed shipping and a dosing guide. Walk away from anything sold without a prescription, labeled for research only, or shipped from outside the U.S.

Is compounded tirzepatide legal?

Yes, when it is prescribed for you. State-licensed 503A compounding pharmacies may prepare tirzepatide for an individual patient with a valid prescription when the prescriber decides a compounded version meets that patient's medical need. It is not FDA-approved.

Is compounded tirzepatide safe?

The active ingredient has a known safety profile, but compounded products are not reviewed by the FDA for quality. The main added risks are dosing mistakes and poorly made products, so use a licensed provider and a state-licensed pharmacy, and follow your label and the syringe your pharmacy sends.

Is compounded tirzepatide the same as the FDA-approved product?

It contains the same active ingredient, but it is not FDA-approved and is not the same as any brand-name medication. It is prepared by a compounding pharmacy for you rather than mass-produced by a manufacturer.

Important safety information

Boxed warning: Tirzepatide caused thyroid C-cell tumors in rodent studies. It is not known whether it causes thyroid tumors, including medullary thyroid carcinoma, in people. Do not use tirzepatide if you or any of your family have had medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.

Common side effects include nausea, vomiting, diarrhea, constipation, stomach pain and decreased appetite, most often while your dose is being increased. Serious risks include pancreatitis, gallbladder disease, low blood sugar (especially with insulin or sulfonylureas), kidney injury from dehydration, and severe allergic reactions. Stop and seek care right away for severe stomach pain, a lump in your neck, trouble swallowing or breathing, or signs of an allergic reaction.

Do not use if you are pregnant, breastfeeding or planning pregnancy, or under 18. Tell your provider about every medication and condition during your visit. This is not a complete list of risks; your provider and the pharmacy will give you full medication information.

Questions

Frequently asked questions

What is compounded tirzepatide?
Tirzepatide activates two gut-hormone receptors, GLP-1 and GIP, to reduce appetite, slow stomach emptying and improve how your body handles blood sugar. Compounded tirzepatide is prepared for you by a licensed U.S. compounding pharmacy from a prescription written by your provider. It is not an FDA-approved product and is not the same as any brand-name medication.
How is tirzepatide different from semaglutide?
Semaglutide acts on the GLP-1 receptor only. Tirzepatide acts on both GLP-1 and GIP receptors. In separate trials of the FDA-approved products, tirzepatide produced larger average weight loss, but both are effective and the right choice depends on your health history, tolerance and budget. Your provider can help you decide.
How does the online visit work?
After checkout you complete a confidential medical questionnaire. A licensed provider in your state reviews it, follows up if they need more information, and decides whether tirzepatide is appropriate for you. Some states require a brief video or live-chat visit. If you are not approved, you are not charged for medication.
What dose will I start on?
Most people start at 2.5 mg once a week for four weeks and step up in 2.5 mg increments as tolerated. Your provider sets your starting dose and your step-up schedule.
Does the price go up when my dose increases?
No. We do not charge by strength. Every dose costs the same, so when your provider steps you up your plan price stays exactly where it started. The only thing that changes your price is the plan length you choose.
What does the price include?
One price covers the provider visit, your medication at any prescribed dose for the plan length you choose, injection supplies and cold-packed shipping. There are no visit fees, membership fees, dose upcharges or shipping charges added at checkout.
Do I need insurance?
No. This is a cash-pay program and we do not bill insurance. You can pay with a debit or credit card, and many HSA and FSA cards are accepted.
What are the side effects?
The most common side effects are nausea, diarrhea, decreased appetite, vomiting, constipation and stomach pain, most often while your dose is being increased. Less common but serious risks include pancreatitis, gallbladder problems, low blood sugar (especially with other diabetes medicines) and kidney problems from dehydration. Tirzepatide carries a boxed warning for thyroid C-cell tumors seen in rodent studies; do not use it if you or a family member has had medullary thyroid carcinoma or MEN 2.
Who should not take tirzepatide?
Do not take tirzepatide if you are pregnant, breastfeeding or planning pregnancy, have a personal or family history of medullary thyroid carcinoma or MEN 2, have had pancreatitis, or are under 18. Tirzepatide can reduce the effectiveness of oral birth control while your dose is increasing; tell your provider about every medication and condition during your visit.
How do refills and cancellation work?
Your plan does not renew automatically. Before each refill your care team checks in with you, and nothing ships or is charged until you confirm. You can pause or cancel from your account at any time with no fees. Federal and state law do not allow medication to be returned once shipped, so all medication sales are final.
Where is this available?
We currently serve 49 states (all except Alaska). Some states have additional visit requirements, which are handled during intake.
