What is ciclopirox and how does it work?
Ciclopirox is a topical antifungal in the hydroxypyridone (pyridinone) class, sold under brand names such as Loprox, Penlac, Ciclodan, CNL8, and Loprox Shampoo. It comes in several formulations for different problems. The 8% topical solution, also called nail lacquer (brand Penlac), is FDA-approved as part of a comprehensive management program for mild-to-moderate fungal nail infection (onychomycosis) of the fingernails and toenails without involvement of the lunula, caused by Trichophyton rubrum, in people with healthy immune systems. The 0.77% cream, gel, lotion or suspension (Loprox) treats athlete's foot (tinea pedis), jock itch (tinea cruris), ringworm (tinea corporis), cutaneous candidiasis, and tinea (pityriasis) versicolor. The 0.77% gel and 1% shampoo (Loprox Shampoo) treat seborrheic dermatitis of the scalp.
Ciclopirox works differently from many other antifungals: it chelates (binds) polyvalent metal cations, which disrupts the fungal cell's metabolism and stops the fungus from thriving.

Ciclopirox is one of several antifungal options, and the right choice depends on what is being treated and where. For nail fungus, a newer prescription topical is efinaconazole (Jublia), and terbinafine is a common oral option; each works differently and has its own considerations that you and your prescriber should weigh.

A few formulation-specific points: the 8% nail lacquer requires professional removal of unattached, infected nail (sometimes as often as monthly), is flammable so it should be kept away from heat and flame, and is not recommended together with systemic (oral) antifungals for the same nail infection. Ciclopirox has not been established for people who are immunocompromised, such as those with diabetes or HIV. The nail lacquer was studied in patients 12 and older, and other forms have their own age limits.
